Our History

The Diabetes and Hypertension Management Foundation (DHMF) was established with a profound commitment to the principle of Ujamaa – the Swahili concept of “familyhood” and community. Founded under the leadership of Duncun Motanya, the foundation emerged from the DMMC WhatsApp Support Groups to create a structured, supportive community for individuals living with diabetes and hypertension.

Our foundation combines traditional community values with modern healthcare management, creating a unique support system that addresses both the medical and social aspects of living with chronic conditions. Based at the DMMC LTD, Ngara Shopping complex in Nairobi, we’ve grown into a vital resource for our members, providing not just medical support but also emotional and economic empowerment.

Our Mission

To foster a strong sense of community, provide comprehensive education and resources, advocate for accessible healthcare, promote preventive measures and early intervention, and empower members economically.

Executive Leadership

Duncun Motanya – Founding Chairperson

Leading the DHMF with vision and dedication, Duncun Motanya established the foundation with a clear focus on community empowerment and comprehensive healthcare support. Under his leadership, the foundation has developed a robust framework for member support and community engagement.

Organizational Structure

The DHMF is governed by a dedicated leadership team including:

  • Chairperson
  • Secretary
  • Treasurer
  • Board of Directors (elected by members)
  • County Group Leaders (representing each county)

Our leadership structure ensures representation at both national and county levels, facilitating effective implementation of our programs and initiatives.

Eligibility Requirements

  • Open to individuals living with diabetes and/or hypertension
  • Must be a member of DMMC WhatsApp Support Groups

How to Join

  1. Complete the DHMF Member Registration Form including:
    • Personal information
    • Contribution details
    • Emergency contact information
    • Member declaration and consent

Membership Requirements

  • Monthly contribution of Kshs 300/= 
  • Payment due by 10th of every month
  • Payment methods: Mpesa or Bank Transfer
  • Benefits become accessible after three consecutive contributions
